McIntosh MCD12000 outputting high pitch Radio Sound

I have recently purchased McIntosh MCD12000 reference DAC. 

When DAC is powered on, it starts a high pitch sound when on USB mode . In fact , it shows mode as USB DSD64 ( no idea why it says DSD64 ) . When I power up the DAC and immediately upon tube warm up, this high pitch/radio / airplane ✈️ sound comes out of each left and right tower . 

Sometimes switching between between the speaker. In fact , if you check the meters on MCD12000 you can see both meters are showing halfway or all the way( sometimes when it switches between towers, I see one meter moving with high pitch airplane sound out of one tower then switching to another tower ) .

The first MCD 12000 was RMA-ed because of this and now I see the second one doing the same .

Again, this has nothing to with the streamer , it will behave like this even if no Streamer is connected . 

Half way workaround : change mode (input) from USB to MCT or another mode. And even this could be temporary. 

This is clearly a software bug , McIntosh is not able to resolve this issue
