Budget and any other constraints, such as size of speakers, can they be floorstanding, and size of room. B&W 303 are good if he needs a small speaker and the room isn't too big.
Since he's a doctor(=$$$...$) look into the top end Cabasse $4K to $13K (a french lab but drivers made in Georgia) as well as Ambience a Australian lab making a solid wood cabinet hybred Ribbon $6K call Darryl in Austin Tx. Digital Systems (he is listed as a Kora Dealer for his p#) You'll only hear of these from me.
If I were doctor on residential practice, I wouldn't buy any speakers since there would be no time to listen to them. Becides there is no money there. My friend gave me his vinyl collection for the period of his residencial practice since he doesn't have any time to listen to them. If he realy needs speakers he has to go to the listening room and listen to different speakers with probably same electronics. Maybe he would just be OK with cheap and small monitors for the beginning.
