Yes, the Manley Steelhead is still relevant. I don´t see how you could do better for the money.A caveat: I believe that the earlier model had QC problems, but these seem to have been overcome.

+1 for @mulveling FR 64 FX and stone-bodied Koetsu is pure magic. Not so sure about FR 64 S and Koetsu, though....

My personal experience is that CJ `s build quality is excellent, and their amps have a very long life. If anything, I would say that CJ equipment is harder on the tubes, with lower tubelife expectancy than many other brands. 
Any experience with Tube rolling 6922 for Conrad Johnson preamp?
AI second that. I haven`t tried the ET series preamps, but the ET 250 S has 2 driver 6922/6DJ8/Ecc 88 tubes. They wear out after about 1000 hours. That said, the best sound I have had was with a pair of Mullard Ecc 88s.

That`s a hard one, depending on your downstream equipment and personal taste. IMO, a wood-bodied Koetsu would be a safe bet. At the moment I have a London Decca Reference in my SME V, and it sounds great. But I don`t think that such a idiosyncrati...
